The The adventure begins with Baron DeCobray, now managing the Vierme estate, revealing that he believes he has discovered the source of the curse and the blight that afflicts the land. He tasks the party with traveling to the Great Library of Winetha to meet with Sage Theron, a scholar who may know more about the curse's solution. The Baron reveals that the "Eye of Water," an artifact stolen from the island of Ooaahhuu by Count Ibilis's great-grandfather, is the source of the curse and the blight. He believes the curse can be lifted by returning the Eye to a statue of the goddess Pele on Ooaahhuu. The party must also bring Count Ibilis or a vial of his blood, as it may be needed for a blood ritual to seal the Eye and restore balance.
After they meet with Sage Theron, the party is directed to a dockside tavern to meet with Captain Bastian, who has agreed to take them to Ooaahhuu. While at the tavern, the party overhears rumors of a ghost ship and an increased number of missing vessels. They are also attacked by bandits, who are either attempting to rescue Count Ibilis or steal the Eye of Water for a mysterious faction known as the "Admiralty". Following the brawl, the party boards Captain Bastian's ship, the Burning Dragon, and sets sail for Ooaahhuu.
During their journey, the party's ship is intercepted by the ghost ship, the Caleuche, which has been corrupted and is no longer fulfilling its purpose of saving souls. The crew and captain of the Burning Dragon vanish, leaving only the party and a mysterious cat, the incarnation of the sea deity Tangaroa. The party must board the Caleuche, which is now under the control of a monstrous Rat King who stole the ship's essence. They must battle the Rat King and his mutated minions in a corrupted version of Fiddler's Green, a tranquil afterlife paradise. By defeating the Rat King and returning the ship's essence, the party restores the Caleuche and earns the gratitude of Tangaroa, setting the stage for the final part of the trilogy.
